Why liquid can remain

The pen accommodates priming and four 0.6 mL doses. The original design leaves visible liquid but cannot dial another complete dose. The modified design reduces the remainder. [1]

The amount you can see is therefore the wrong basis for a treatment decision. “There is liquid” and “there is a complete, accurately deliverable dose” are different statements. A cartridge photograph cannot tell you to add another treatment week, and another person's experience does not change your pen's instructions.

The nickname “golden dose” describes an online practice, not a manufacturer-authorised dose or a separate strength of Mounjaro. Use the four-dose statement when discussing supply, price or a concern with your pharmacist.

The changed design does not change the prescription

Both designs work the same way and provide four doses. Less residual liquid does not mean fewer treatments. [1]

If the plunger position or appearance differs from a previous pen, check the leaflet supplied with the new pack and ask the dispensing pharmacy to identify it. Keep the carton available during that conversation. Avoid deciding that a pen is fake or defective solely because it does not match a social-media photograph.

A useful question is: “Is this the correct presentation and strength for my prescription, and can you show me its instructions?” That gives the pharmacist something specific to check, rather than asking them to interpret an unofficial click chart.

Why not recover the remainder?

The UK prescribing information instructs users to discard excess solution after the four doses. The MHRA also advises against extracting remaining medicine. These instructions provide no validated method for a fifth injection, partial top-up or syringe conversion. [3][2]

Converting between milligrams, liquid volume and syringe markings would also require reliable knowledge of the actual concentration and amount delivered. An online calculation cannot establish that an improvised procedure is accurate or appropriate. We therefore do not provide extraction steps or a “fifth-dose” calculator.

The same principle applies to the phrase “just a little extra”. If you are unsure whether you received the full dose, uncertainty should lead to a pharmacist or prescriber check, not a guessed addition.

If a dose seems incomplete

Follow the troubleshooting instructions in the leaflet for your own pen. If a problem remains, contact the dispensing pharmacy or prescribing service before taking more medicine. Keep the pen available for their advice and record what happened, including the dose selected and whether the device behaved as expected. [4]

Useful details include the date, prescribed strength, dose number within the pen, and what you saw in the dose window. Distinguish a missed treatment from an uncertain injection: they are different situations for the clinician to assess. Do not use residual volume as proof of how much entered your body.

If you already used an extra or uncertain amount, seek prompt professional advice and explain the product, timing and what was done. The patient leaflet advises contacting a doctor promptly after using too much. [4]

Calculate supply using labelled treatments

One pen supplies four weekly treatments over 28 days. Four weeks is not the length of every calendar month. [1]

For illustration, a hypothetical £160 pen costs £40 per labelled weekly dose: £160 divided by four. Dividing by five would produce £32, but that figure depends on counting a treatment the pen is not intended to provide. This arithmetic is an illustration, not a current price quote.

Plan repeat supply around the fourth labelled treatment. Include delivery lead time and any review the service requires. If affordability is becoming a problem, raise it before running out so that the prescriber can discuss legitimate options.

Use the labelled storage and disposal rules

Discard after four doses or 30 days from first use. Follow the leaflet's storage instructions and pharmacy's disposal advice. [1][4]
